Care Giver Endorsement

In consideration of the premium charged, it is hereby understood and agreed that the definition of Professional Services is amended to include:

Care Giver

Section V. DEFINITIONS is amended to include the following:

" Care Giver " means one who provides the following services to others for a fee under a supervised plan of care:

meal preparation;

transportation;

running errands;

ensuring medications are taken;

companionship, observation, and activities to stay engaged; and

accompanying to doctor appointments.

The services performed by a Care Giver do not include:

providing actual health care services directly; or

performing any services in any trade, business or profession other than a Care Giver , including without limitation activities as a nurse or other health care provider.

All other terms and conditions of the Policy remain unchanged.
