Independent Contractor Care Manager Endorsement

In consideration of the premium for which this Insurance is written it is hereby understood and agreed that the individuals listed under Forms & Notices on the Declarations qualify as additional assured(s) under the Policy with respect to work performed in the capacity of a Care Manager for or on behalf of the Named Assured .

All other terms and conditions of the Policy remain unchanged.
